Metabase alert to Telegram

We want to send Metabase Alert to Telegram. Please help to create this integration. I will use it in my office, and we will buy the subscription if you can provide it fast.
Here below Our requirements :
In Metabase there is Alarm feature that will send jpg of the Question to email if the goal is reach in the first time.
- I need this Alarm send to Telegram Group. so our team can notice.
- However usually the alarm email Metabase only send the picture of the Question's Graph. but for this task We need to add some text in the message as well. so the team will understand better.. Please add text input during the setup of the alarm forwarding which is different for each Question.
- The Alarm forwarding should be sent to different group telegram refer to the title of the Question
