I started writing this myself, and it works ok. Images are a pain. This would be a great product to get a company behind. I'd buy it.

In my case I assigned each heading level to a work item type. Then used Field: Text type notation in word to indicate what field I am updating. Parent/Child relations were maintained and could be moved around by moving a heading from one spot to another.

My solution is not all that great from a code perspective and I do not have time to maintain it. But I would find great value if someone else did this.
